PeaceHealth is seeking a Nurse Practitioner or Physician Assistant to join a vibrant multi-disciplinary Hospice Care team in Vancouver and Longview, WA.

PeaceHealth's Hospice Care program provides a full continuum of care for patients and their families. Medical, spiritual, and emotional assistance helps patients and their families to ease the symptoms of life-threatening illnesses. This full-time role will require traveling in Northern Clark County and Cowlitz County. Ideal candidates will have a certification for Hospice/Palliative Care.

  • On-site complex case management team, social workers, chaplain, and partnerships with the Ray Hickey Hospice House

  • Primarily outpatient. Opportunity to grow service-line within the region.

  • Schedule: Work four days weekly, with 2-6 face to face visits daily

  • Call: 1:4 one weekend night of call in rotation
