Coordinates the implementation of Population Health management initiatives across a network of practices within a region enabling primary care physicians to deliver higher quality care. Supports IU Health management teams, regional physician leadership, primary care providers, and practice staff across assigned regions of Indiana to integrate analytic, ancillary, and operational capabilities to provide better health outcomes, lower cost of care, and improve patient and clinician experience. Assists the designing and monitoring of population health metrics while maintaining strong relationships with providers and staff as well as supports value-based care concepts, initiatives and strategies.

Qualifications For The Population Health Intermediate Consultant Role

- Bachelor's degree in business, healthcare administration, or a relevant field required. Master's degree preferred.
- 3-5 years of relevant experience required.
- Proven ability to implement data into practice; respond to analytical insights with strategies and solutions that impact the provision of care; evidence-based decision-making skills.
- Reliable transportation is critical as travel to multiple locations within a specific region of Indiana is required; travel estimated at 35% - 50% (no overnight travel anticipated).
- Must have relevant experience working with physicians including the ability to leverage data to influence physician and physician practice behavior.
- Must have prior experience managing a Profit and Loss statement.
- Requires basic proficiency in Microsoft Office (Word, PowerPoint, Excel).
