Job Description

  1. Primarily responsible for handling cases with varying levels of complexity while utilizing appropriate clinical assessments and advanced knowledge to provide an analysis and make appropriate recommendations. 2. Works independently and consistently exercises discretion and sound judgment in conducting annual assessments of the residents' needs using the prescribed resident application. 3. Coordinates the annual building assessment to identify residents who are frail and/or at risk. When necessary, assists management to permanently place a resident in a more appropriate setting after utilizing the Chicago Housing Authority's standardized Senior Needs Assessment.4. Researches and maintains a network of service providers in the community.5. Assists residents in the development of their annual service plans, including but not limited to providing referrals to service agencies and providers and monitoring that the resident receives the services. Additionally, is also primarily responsible for working independently and consistently exercising discretion and sound judgment in making recommendations on what services the residents need on an as needed and ongoing basis. 6. Completes all required documentation in accordance with agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies;7. Plans, advertises and facilitates a minimum of 24 program events , activities, and educational presentations for the residents and their families on topics including but not limited to health, wellness, safety, financial assistance and resident rights.8. Handles all resident and visitor complaints and crisis situations in consultation with their supervisory chain in compliance with Agency/Programmatic policies and procedures and treats each case with sensitivity and expedience.9. Assigns the volunteer Senior Community Ambassadors in the building daily tasks in accordance with Senior Community Ambassadors Program (SCAP) guidelines. 10. Assists with the training and general oversight of staff, as directed by their supervisor.

PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned:Lift up to 25 pounds.Climb up and down up to 7 flights of stairs at a time. Push and pull objects up to 25 pounds Kneel and move from sitting, bending, kneeling or standing multiple times a day.

Prefer bi-lingual Korean

Education and Experience Requirements:

Preferred Relevant Education: MSW

Minimum Relevant Education: Master's degree in Social Work, Counseling, or Geriatrics or in a related field of study.

Preferred Relevant Experience: 2 - 5 years working with seniors

Minimum Relevant Experience: 1 - 2 years working with seniors
