What Youll Be Doing
ED to Home is a medical and social intervention designed to shift acute and urgent care delivery from the hospital to the home using physicians, community health workers, and the delivery of home-based medical services. The Medical Director of ED to Home, reporting to the Senior Vice President of Medical Affairs, provides leadership for and development and clinical and operational oversight of ED to Home and the Complex Care Consult Service at hospitals across Massachusetts. The portfolio includes the ED to Home programs at Boston Medical Center (BMC), Baystate Medical Center, North Shore Medical Center, and Beth Israel Deaconess Medical Center and the Complex Care Consult Service at Boston Medical Center (BMC), as well as the development of relationships and partnerships with hospital leadership at these institutions and others to improve the cost and quality of care for hospitalized CCA members.

This position is responsible for providing clinical and operations leadership of ED to Home and the Complex Care Consult Service, working closely with the hospital-based clinical providers, outpatient clinical care teams, care delivery partners, CCA technology and operations staff, and the transitions of care team to ensure the best inpatient care and smoothest transitions possible for the members who have acute care needs. S/he will be responsible for program development, hiring medical staff, and scheduling and supervising the clinicians and staff. S/he will work closely with the Clinical and Innovations Teams to interface with hospitals across the state (and nationally) to build future Complex Care Acute Services, with a desire and need to augment acute and urgent care services in the home, such as hospital at home and community paramedicine. S/he will develop KPIs and metrics for success, in line with the financial goals of CCA the payer, the clinical standards and objectives of CCA the provider, and the business operations.

Medical and Clinical Responsibilities

Direct Patient Care
The direct patient care component to this role involves direct care on the inpatient service. Additionally, the Medical Director will be available to telephonically to triage calls and discuss cases with other clinicians on the care team during business hours.

Medical Supervisory and Clinical Leadership:

- Interfacing with PCPs, specialists, and hospitals to promote an understanding of CCA's unique model and facilitate collaboration and shared decision-making

- Hiring and Supervision of hospitalist team

- Monitoring, evaluation, and quality improvement activities

- Acting as liaison between CCA and instED and other services/partners that deliver urgent and acute care for CCA members

- Developing educational content around complex care delivery for inpatients geared at CCA clinicians

- Engaging with CCA's acute and post-acute care team to improve quality of care and enhance relationships with SNFs and hospitals caring for CCA members

- Presenting and publishing, specifically around Complex Care inpatient medicine and innovations
Business Operations Responsibilities

- Operating ED to Home such that it could be a self-sustaining entity, with advanced financial, operational, leadership, and business development knowledge and skills

- Actively participating in CCA's leadership committees and other roles as appropriate and as requested by SVP of Medical Affairs, SVP of Innovation, or Chief Medical Officer

- Acting liaison between the clinical and operations teams around issues of hospitals and hospitalization, acute and urgent care delivery, and clinical and financial outcomes

- Familiarizing oneself with the business and financial operations of CCA as they pertain to acute and urgent care

- Employing technology, analytics, and other staff to appropriately support the acute and urgent care programs

- Familiarizing oneself with the innovations and technology potential for acute and urgent care programs, and helping CCA to be on the forefront of these innovations

Essential Duties & Responsibilities

- Function as clinical oversight for the acute and urgent care teams, available Monday - Friday 8 a.m. - 6 p.m. to provide back-up clinical support

- Documents all clinical work in a timely manner and provides the necessary documentation required for data collection and billing purposes

- Participates equally in all call responsibilities

- Serves as medical director for appropriate functional areas

- Leads clinical team meetings and other mandatory educational sessions

- Conducts educational and training activities that promote appropriate and effective patient care

- Provides educational updates to clinical staff

- Participates in hiring, orienting, and training new employees as required

- Participates in quality assessment and quality improvement within CCA as a whole

- Seeks to maintain a constructive work environment and maintains effective communication with other employees and managers

- Provides clinical oversight of the acute care team staff and develops productivity, quality, and performance metrics

- Communicates with primary care teams and coordinates post-acute follow-up as needed when providing direct care, and ensuring these standards are met by his/her staff

- Participates in case review and quality improvement projects

- Participates in annual clinical team retreats and social / teambuilding events

- Maintains required professional credentialing and appropriate CME standards

- Attends external meetings and activities as a representative of the organization as requested

What Were Looking For

- MD or DO degree from an Accredited institution

- Board Certified or Board Eligible in Family Medicine or Internal Medicine

- Interest and experience in serving complex, vulnerable, and disabled populations is essential

- Experience with quality improvement, monitoring and evaluation, and health systems strengthening, innovation, and training is desired, although not essential

- Proven skills, knowledge base, and judgment necessary for independent clinical decision-making

- Excellent organizational, time-management, and problem-solving skills

- Ability to function effectively as part of a multi-disciplinary team and to independently develop and execute an agenda

- Effective oral and written skills

- Strong interpersonal skills

- Commitment to social justice in medicine and an understanding of healthcare reform
